Work Hours: Monday - Friday: 8:00 am - 5:00 pm

You made a decision to join the healthcare industry. You excelled through school and became a Licensed Practical Nurse or Registered Nurse. After graduation, you quickly realized it was more about caring for as many patients as fast as you possibly can and you do not having the time to stop and make a positive impact in a patient's health. ChartSpan is revolutionizing chronic patient care and we want you to be a part of our team. Our Patient Care Supervisor helps patients with chronic illnesses navigate through the confusing world of healthcare and assists them in obtaining the resources they need to improve their health. You will also supervise and provide leadership and training for clinicians and health coaches as they coordinate care for patients.

What we are looking for in a candidate:

  • A passion for helping others
  • Excellent verbal communication skills
  • Previous experience providing leadership and management to other members of a clinical team
  • Competency in typing and data entry
  • Possess a current license as Licensed Practical Nurse or Registered Nurse

You must successfully pass a background check. You must also pass a typing test with 40 WPM speed and 90% accuracy.
