A Lead Triage Nurse coordinates care for patients; providing an essential service which helps providers stay in touch with and meet their patients' healthcare needs in-between office visits. Your role is to support and assist patients in obtaining the resources they need to improve upon their health, happiness, and longevity.

 

You will spend a majority of your time speaking with and relating to patients over the phone to garner a more comprehensive understanding of their needs and concerns. Our patients have a lot of urgent needs that require rapid responses, and that is where you come in! You will be able to use your nursing skills to help many patients, all with telephonic care. *Complex concerns are escalated to practice physicians 

 

Must be available to work the following Triage Nurse shifts

  • Monday - Friday, 8am - 5pm three day per week and 9am - 6pm two days per week

  • One or two Saturday 8am - 6pm shifts per month (with a weekday off)

  • On call one or two weeks per month 




Duties and Responsibilities

  • Triage Services

    • Responsible for the systems and processes surrounding our triage protocol 

    • Coordinate care regarding patient medical issues and concerns

    • Supervise triage services by training and overseeing all nurses utilizing triage protocol

    • Work with triage vendors to create accounts and communicate changes, questions, or resolutions

  • Triage Leads

    • Ensure night shift Triage Lead team is trained using proper triage escalation protocol

    • Download notes from Triage Vendor, add to patient charts in CCM, and follow-up with patients/providers as needed 

    • Input data from Triage Leads into spreadsheet daily

    • Be on-call during assigned Triage Lead shifts during the 1 or 2 nights during the month to answer questions as needed

  • Nurse Urgent Queue

    • Review and complete Nurse Urgent To-Do's, with a goal to keep the queue below 50

    • Communicate with patients and providers to assist in answering questions, coordinating care and services. (examples: schedule appointments, contacting service providers, sending notifications to healthcare providers, calling the patient back to follow up on their recent urgent or emergent medical issue, etc.)

    • Check Notifications in CCM 

  • Lead Nurse

    • Oversee a team of nurses

    • Monitor daily attendance, break and approved Paid Time Off (PTO) requests

    • Review and ensure payroll timesheets are accurate for each team member

    • Ensure consistent follow up and communication.

    • Ensure all agents are compliant with HIPAA and Medicare Fraud, Waste and Abuse rules and regulations at all times 

    • Respond and communicate promptly to all inquiries received via email, text and phone.

    • Establish process controls to ensure all SLA's and KPI's are consistently met.

  • Attend meetings as required of the position

  • Other duties as assigned

Skills and Qualifications

  • Possess a current license as an RN or NP

  • Within six months will obtain multi-state licenses of ChartSpan footprint

  • Excellence in following clinical triage protocols and procedures, and documenting accordingly in patient charts

  • Demonstrate sensitivity and empathy to chronically ill patients

  • Demonstrate excellent verbal and written communication skills

  • First-rate time management and organizational skills

  • Ability to work Triage Nurse shift

  • You must successfully pass a background check
